WAIVER AND RELEASE OF LIABILITY

I {name} hereby agree to the following:

That I am participating in the training, programs, exercises and events offered at Fairbanks Barbell Club, 1256 Queens Way, Fairbanks Alaska 99701, during which I may receive instruction.

I understand that it is my responsibility to consult with a physician prior to and regarding myparticipation in any fitness or exercise program. I represent and warrant that I am physically fit and I have no medical condition that would prevent my full participation.

In consideration of being permitted to participate in any fitness or exercise program I agree to assume full responsibility for any risks, injuries, or damages, known or unknown, which I mightincur as a result of participation. In consideration of being permitted to participate in any fitness or exercise program I knowingly, voluntarily, and expressly waive any claim I may have against Fairbanks Barbell Club, concessionaires, owners, landlords, or insurers for injury or damages that I may
sustain as a result of participation or experience at or around Fairbanks Barbell Club, 1256 Queens Way, Fairbanks Alaska 99701.

I, my heirs, and/or legal representation forever release, waive, discharge, and covenant not to sue Fairbanks Barbell Club, concessionaires, owners, landlords, or insurers for any injury or death cause by their negligence or other acts.

I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
